SYMPTOM Line output transistor fails repeatedly.
CAUSE
Poor slow start and drive signal fluctuations.
ACTION
Add the following components to the underside of the PWB.
1.   Connect a 2k7, 1/8W resistor between the cathode of D1001 and ground.
2.   Connect a 47nF, 63V capacitor between the anode of D1001 and ground.
Ensure that the line output transistor is the type listed below.
Use the part numbers listed below.
