Failure of Q509.
CAUSE
Excessive excursions of the positive forty volt supply from the line stage.
ACTION
Fit an extra zener diode, D632, in the position of R626 as shown in the diagram below.
Note that this diode has been fitted during production as of the following serial
numbers.
